*First and Second No Show will be documented in your chart without consequences and remain documented for one year. Every subsequent No Show will remain marked in your chart for one year.

Third No Show or Last-Minute Cancellation:

Non-Medicaid Funded Visits:

A $60 fee will be applied to your account for any no show or last-minute reschedule/cancelation.

Medicaid-Funded Visits:

Patients who miss an appointment or reschedule/cancel at the last minute will be restricted to making same-day appointments only for the following *three months, starting from the date of their last missed appointment

Recurrent No Show or Last-Minute Reschedules/cancelations:

Patients who have *four no shows or last-minute reschedule/cancellations within a one-year period may become ineligible to receive care at Prism Family Medicine.

To improve appointment access and lower costs, we've set a policy for no-shows and last-minute changes. A last-minute change is any appointment change made within three hours of the scheduled time.
